President Coin was definitely the person most responsible. She knew she'd need Katniss' support to have any hope of ruling after Snow, so she set up Prim to die because she thought Katniss would support her in grief. She sent Prim to join the medics on the front line even though Prim was under age, and sent a stolen Capitol hovercraft to drop bombs on the children who were forming Snow's human shield. The medics, including Prim, were then killed by a second explosion from the bomb.
Gale was involved too because it was he and Beetee who'd devised these double exploding bombs, and the idea of using them this way on enemy medics. They didn't realise their own medics would also try to help and get caught up. Gale came up with the idea of the trap while Beetee did the technical side.

It was Gale's fault that she died because he had been working on the bomb that killed Prim and he knew she would be out there helping as a nurse, but his anger at the Capitol took over and I feel he really didn't care.
