I think he's really dead (like how Dean has died before) but they're going to bring him back somehow. I mean, you can't sustain a wound like that and not at least hover between life and death like Dean did. So I say he's really "dead" - for all intents and purposes - but of course, they're going to bring him back.

Maybe some of the third season will be about how Dean brings Sam back from the dead? I don't know, maybe they'll pull a Buffy and yank Sam outta hell or something.
