On your profile, you click the little button-thingy near your icon that says "upload photos," and just choose what you want to upload.

On a club, you go to the images section. You go to whatever category your picture fits under(photo, wallpaper, icon,fan art...) and click "add photo," or whatever your image is.

Also, you don't need to type anything into the box. Just click the "submit image" button and choose your file's name.
