well all of the above but religion sort of includes the other 2... it's just that religion and some people's interpretation of the bible is the cause of most homophobia, people say that god hates fags or whatever :/ if it weren't for religion the world would have so much better liberal laws and opinions!

All of them tbh but if I'd have to pick one, I'd say the bible. Also, isn't religion + God + the bible basically the same source in terms of people using one's faith against homosexuality?

Well actually I believe those comments in the Bible are mis-translations; I don't believe the Bible is actually against homosexuality or anything like it. But that's just my opinion based on sources I've heard and read about.
