He turns into a full Hollow during his fight with Ulquiorra, which is shown in two episodes... 271-272, I believe. He also turns into the same Hollow form during Bleach: The Hellverse/Hell Chapter.
Another full Hollow form he turned into was during the Gotei 13 Invading Army Arc, where during about 3 episodes he is shown as a full Hollow, but a different form to the fight against Ulquiorra. Glimpses of this same form are also shown briefly in the Tales of the Zanpakutous arc.
